Community Health Needs Assessment
A Comprehensive Community Health Needs Assessment (CHNA) provides the general public and policy leaders with data and information on the local health status of the communities we serve and existing resources to address health issues. The following documents summarize the most recent CHNA reports conducted with collaborating partner(s) as listed.

The CHNA uses a community-driven approach that supports the hospital in planning and carrying out community-based programming to more effectively handle the health needs of underserved and vulnerable populations.
The in-depth reports will help the hospital and community make decisions about the future of healthcare and prevention in their areas by identifying how the community compares to the state on many dimensions of health. The findings point out several major health issues facing our community, including levels of obesity, access to health care and mental health services, and chronic disease care (e.g. diabetes, heart disease and respiratory disease).
